About this file
This is a Sensitive Information Destruction Confirmation Affidavit attachment related to a Statement of Need issued by the Georgia Department of Community Supervision (DCS) for a security assessment project. The affidavit is required as part of the formal proposal submission process for contractors responding to Request for Qualified Contractors (RFQC) 98000-00000-3088 and the subsequent informal bid solicitation Statement of Need 98000-00000-5619. By executing this notarized document, suppliers must confirm that all sensitive information in their custody has been destroyed within seven days of proposal submission or immediately upon demand by the Georgia Technology Authority (GTA), whichever occurs first. The document establishes destruction standards requiring that electronic sensitive information comply with NIST SP 800-88r1 Guidelines for Media Sanitization, while hardcopy and printed materials must be destroyed using cross-cut shredders or equivalent methods.
The affidavit requires completion and notarization by an authorized representative of the supplier's company, including the legal company name, representative signature, printed name, title, address, and notary public certification. This destruction confirmation is a mandatory compliance requirement for all contractors bidding on the DCS security assessment contract, which aims to evaluate the department's cybersecurity incident response capabilities and IT security control implementations. The form provides multiple blank spaces to document specific destruction methods employed, allowing contractors to detail the equivalent destruction methods used for sensitive information handling beyond the specified shredding and NIST-compliant electronic sanitization approaches.
