About this file

This document is a Past Performance Questionnaire template designed for evaluating a contractor's performance across multiple criteria. The form provides a structured framework for assessing a contractor's performance using adjectival ratings in seven key evaluation areas: Quality of Work, Personnel, Cost/Budget Control, Customer Satisfaction, Timeliness of Performance/Deliverables, Business Relations, and Overall Performance. The accompanying rating guidelines offer detailed descriptions for each performance rating level, ranging from Unsatisfactory to Exceptional, with specific guidance on how to evaluate factors such as contract compliance, cost management, responsiveness, and technical excellence. The questionnaire allows the evaluating agency to provide comments, assign ratings, and indicate whether they would select the contractor again, serving as a comprehensive tool for documenting and analyzing contractor performance.
