About this file
This document is a Past Performance Questionnaire for a Department of Veterans Affairs (VA) contract opportunity. The form is designed to collect detailed performance feedback from previous clients about a contractor's performance, with ratings across multiple evaluation categories including management, quality control, regulations compliance, problem resolution, schedule adherence, and warranty response. The questionnaire allows reference contacts to rate the contractor on a 5-point scale from Exceptional (5) to Neutral (1), with instructions to provide narrative explanations for ratings of 3 or lower.
The related federal contract opportunity is a Design-Build construction project (Solicitation 36C24825R0169) for refreshing Lake Baldwin Pharmacy Finishes, issued by the VA Veterans Health Administration Veterans Integrated Service Network 8. The project requires comprehensive design and construction services, with a NAICS code of 236220 (Commercial and Industrial Building Construction) and a Product/Service Code of Z1DA (Maintenance of Other Hospital Buildings). The period of performance is 240 calendar days after Notice to Proceed, and contractors are expected to provide all labor, materials, tools, equipment, and design-build services necessary to complete the project as specified in the Statement of Work.
