About this file

This is an Attachment F Certified Disadvantaged Business Outreach Plan form for RFP S-DASOBO-00017122, a Conservatorship Investment Advisor Services procurement issued by the State of Oregon through the Department of Administrative Services (DAS). The form requires proposers to document their outreach efforts to certified firms and identify subcontracting opportunities. Proposers must indicate their own certification status under Oregon Revised Statute 200.055 by the Oregon Certification Office for Business Inclusion and Diversity (COBID), including designations as disadvantaged businesses (DBE), minority-owned businesses (MBE), woman-owned businesses (WBE), service-disabled veteran-owned businesses (SDV), or emerging small businesses (ESB). The form requests submission of lists of certified firms with which the proposer has had contractual relationships within the preceding two years, as well as firms that may qualify as minority-owned, woman-owned, service-disabled veteran-owned, or emerging small businesses but lack formal certification.

For proposers who anticipate subcontracting opportunities, the form requires detailed documentation of steps to solicit certified firms, a description of anticipated subcontracting opportunities with approximate dollar values, and confirmation of willingness to report subcontractor identity and value information to COBID upon contract award. The outreach plan information is not considered in the scored evaluation of proposals. All certified firms must be provided an equal opportunity to compete for and participate in subcontracts resulting from this procurement, as required by state funding regulations. Proposers without anticipated subcontracting opportunities need only submit the first page of the form with their proposal.
