About this file
This is a Price Workbook (Attachment F) for the State of Illinois Department of Public Health (IDPH) Immunization Information System Procurement, designed to standardize cost proposals across all offerors. The workbook establishes a costing model for implementation deliverables, ongoing costs, and vendor hourly rates aligned with a ten-year contract term. Offerors must use this workbook to calculate prices and submit it with their complete offer through the BidBuy system. The workbook captures one-time costs for deliverables across multiple service domains including Project Management, Validation Requirements, Solution Design and Development/Configuration, Data Migration, Testing, User Training, Solution Hosting & Provisioning, Cutover/Deployment/Hypercare, Maintenance and Operations, Knowledge Transfer and Exit Transition, Multi-Vendor Integration, and Organizational Change Management. All functionalities selected as solution methods in the Traceability Matrix must be included in the offer and reflected in the price proposal.
Pricing must be structured through five integrated worksheets: the Total Price Summary (which calculates overall cost across 120 contract months), Deliverables Cost (capturing one-time implementation costs and level of effort in human work hours), Ongoing Cost (itemizing recurring costs such as hosting, provisioning, and maintenance aligned with the project workplan), Price Assumptions (documenting all pricing assumptions, expectations, and vendor performance parameters with references to specific offer sections), and Blended Rate Card (establishing hourly rates for key vendor roles including Project Manager, Functional Manager, Technical Manager, OCM & Training Manager, Business Analyst, Solution Architect, HL7 Integration Specialist, and others, with composition weights and year-over-year increases for the ten-year term). The workbook includes 400 annual enhancement hours available to IDPH on a rolling twelve-month basis beginning with the start of maintenance and operations services, with unused hours rolling over to subsequent periods. All offerors must ensure formula accuracy and proper rollup into the Total Price Summary prior to submission, establishing an apples-to-apples comparison basis for evaluation.

| Introduction & Instructions |
| This Price Workbook contains multiple worksheets designed to provide an understanding of the costing model for this RFP. Offerors are to use this workbook to calculate the values of implementation deliverables, ongoing costs, and key vendor role hourly rates which align with its Offer. All costs associated with an Offer to IDPH must be tied to either a Deliverable (worksheet 2) or Ongoing Cost (worksheet 3). Please refer to the RFP for all necessary details regarding scope, scale and complexity of this solicitation, and establish the price proposal accordingly. All functionalities for which an Offeror has selected a Solution Method other than “None” in column C of Table 2 and Table 3 in Attachment E, Traceability Matrix, worksheet F.3. Desirable Elements, must be included in the Offer and reflected in the price proposal. In addition to these instructions, there are more instructions and guidance provided within subsequent worksheets of this workbook. Cells requiring Offeror data entry are in light-green highlight as shown for cell B9 above. It is the responsibility of the Offeror to ensure the accuracy of the Price Workbook formulas prior to submission. |
NOTE: It is important all Offerors understand that price proposals must be developed using this workbook, and uploaded to the Attachments Tab in BidBuy as instructed in section C.1. and F.6. of the RFP. Utilization of this workbook ensures all Offerors are pricing Offers in the same manner leading to a truer apples-to-apples comparison. Details within this workbook will also support stronger management of the potential future contract; therefore, this workbook shall be submitted with the complete offer.

The Annual Period is defined as a rolling twelve (12) months, not a calendar year.
1. Column J: This column identifies the Annual Period to which the cost applies.
2. Column K: The Offeror shall specify the month of the contract when Annual Period 1 will begin (K11). This month should align with the start of M&O services as noted in the workplan submitted with the Offer. All other Annual Period months will auto-populate based upon the month selected for Annual Period 1.
3. Column L: The Offeror shall specify the year of the contract when Annual Period 1 will begin (L11). This year should also align with the start of M&O services as noted in the workplan submitted with the Offer. All other Annual Period years will auto-populate based upon the year selected for Annual Period 1.
4. Column M: This column identifies the Annual Hours available for use at the discretion of IDPH for the design, development/configuration, implementation, and hypercare support of any enhancements to the Solution. IDPH does not guarantee the number of hours used annually.
5. Column N: This column will auto calculate the Annual Cost, leveraging the blended hourly rate of all Key Vendor Roles for Year 1 as calculated in worksheet 5. Blended Rate Card, cell E26.
Annual Hours will be used for any enhancement Task Order(s) until exhausted. Hours needed to complete deliverables associated with a Task Order will then leverage the appropriate contracted blended rate. Any unused Allotted Annual Hours not used by IDPH shall roll-over to the next Annual Period, and continue rolling over into subsequent Annual Periods, unless otherwise agreed upon.

| Information & Instructions | Add % |
| 1. Column A: Key Vendor Roles identified in section A.2.2. of the RFP have been prepopulated. |
2. Column B: Offerors must enter the resource level (Sr./Jr.) whom has been identified to fill each role within the Offer.
3. Column C: Offerors must enter the composition weight of each role. Composition Weight is defined as the percentage contribution of each role to the overall cost per hour by calculating the proportion of total costs each role represents.
4. Column D: Offerors must enter the Year 1 hourly rate for each role.
5. Cell G8: Offerors must enter the year-over-year percent increase for hourly rates.
6. Hourly Rate for Year 2 through Year 10 will auto calculate using the year-over-year increase entered into cell G7.
7. Composition Rate will auto calculate by multiplying the hourly rate by the composition weight.
8. Blended Composite Rate, shown in columns E, G, I, K, M, O, Q, S, U, and W of row 25, is derived by summing the products of each role's composition weight and their respective hourly rates, providing a single blended rate. The blended composition rate will be calculated using only key vendor roles proposed within section A.2.2. of the RFP.
NOTE: Offerors proposing key roles beyond those listed below shall use Table 2. Managed Services Rates for Additional Roles to identify these roles. Offeror shall populate the light green cells using the same instruction as provided for Table 1.
