About this file

This is a Certified Disadvantaged Business Outreach Plan form (Attachment F) for procurement S-34000-00012445, required by the Oregon state government for proposers seeking state-funded contracts. The form requires proposers to provide information about their certification status under ORS 200.055 by the Oregon Certification Office for Business Inclusion and Diversity (COBID), including whether they hold certifications as minority-owned businesses (MBE), woman-owned businesses (WBE), service-disabled veteran-owned businesses (SDV), disadvantaged business enterprises (DBE), or emerging small businesses (ESB). Proposers must list their contractual relationships with certified firms from the past two years, identify potential subcontracting opportunities, and describe their outreach plans to solicit certified firms if awarded the contract.

The form emphasizes that certified firms must have equal opportunity to participate in state-funded contracts, and proposers must certify they have taken reasonable steps to ensure such participation. The information submitted will not be considered in scored evaluations, indicating this is a compliance requirement rather than a competitive factor. If proposers foresee subcontracting opportunities, they must complete additional sections describing their solicitation methods, approximate dollar values of subcontracts, and their willingness to report subcontractor identities and values to COBID. The form serves as both a certification tool and planning document to promote diversity and inclusion in state procurement processes.
