About this file

This is a Certified Disadvantaged Business Outreach Plan attachment for an Invitation to Bid issued by the State of Oregon, acting through the Oregon Health Authority and its Oregon State Public Health Laboratory, for environmental monitoring equipment and service (ITB#S-44300-00016250). The document requires bidders to certify their compliance with state requirements regarding equal opportunity participation for certified disadvantaged business enterprises in contract performance. Bidders must identify themselves as Oregon-certified firms if applicable, including certifications as disadvantaged business enterprises (DBE), minority-owned businesses (MBE), woman-owned businesses (WBE), service-disabled veteran-owned businesses (SDV), or emerging small businesses (ESB), and provide their Oregon State Certification Number.

Bidders must document their previous contractual relationships with certified firms and other potentially minority-owned or veteran-owned businesses from the past two years, and identify any anticipated subcontracting opportunities with corresponding dollar values. If subcontracting is anticipated, bidders must describe the steps they will take to solicit certified firms for those opportunities and confirm their willingness to report subcontractor identities and values to the Oregon Certification Office for Business Inclusion and Diversity (COBID) upon contract award. The submission of outreach plan information is not considered in the evaluation process.
