About this file

This is a Past Performance Questionnaire template issued by the Federal Protective Service (FPS), Western Consolidated Contracting Group, for use in evaluating contractor performance on RFQ 70RFPW26QW9000004 regarding Intrusion Detection System (IDS) installation at the Robert T. Matsui United States Courthouse in Sacramento, California (Building CA0306). The questionnaire is designed to be completed by individuals responsible for administrative oversight of past projects, such as security specialists, Contracting Officers' Representatives (COR), or COTR personnel, and multiple responses are permitted if appropriate. Completed questionnaires should be emailed to Dolor E. Alegrado at Dolor.E.Alegrado@fps.dhs.gov.

The questionnaire requests identifying information about the respondent and the referenced past performance contract, including contractor name, project title, period of performance, and total project value to date. It contains nine performance evaluation sections requiring narrative comments on: contractor professionalism and attentiveness; customer service and responsiveness; quality control effectiveness; problem identification and resolution capability; supervision and staffing adequacy; responsiveness to owner and tenant concerns; overall project management; handling of complex requirements; and willingness to rehire. The final question uses a three-tier rating system for overall performance quality: Highly Acceptable (risk substantially less than competent vendor baseline), Acceptable (risk no more than competent vendor baseline), or Unacceptable (risk greater than competent vendor baseline). Both handwritten and typed responses are acceptable for this March 2016 version template.
