About this file
This is a HUD Form 50070 Certification for a Drug-Free Workplace, issued by the U.S. Department of Housing and Urban Development. The form serves as a certification requirement for applicants receiving federal grant funding through HUD programs. The applicant must certify commitment to maintaining a drug-free workplace across all sites where work is performed in connection with HUD-funded programs or activities. The certification requires the applicant to identify all work performance sites, including street addresses, cities, counties, states, and zip codes on separate attached sheets. The form does not specify response dates, bid submission deadlines, site visits, or bidder conferences, as it is a compliance certification rather than a solicitation document.
The certification obligates the applicant to publish workplace drug policies, establish ongoing drug-free awareness programs, distribute policy statements to all employees engaged in grant performance, and notify employees of conviction reporting requirements and potential disciplinary actions. The applicant must notify HUD within ten calendar days of receiving notice of any employee drug statute conviction and take appropriate personnel action, including potential termination or mandatory participation in approved rehabilitation programs, within thirty calendar days. An authorized official must sign and date the certification under penalty of perjury, with false statements subject to criminal and civil penalties under federal law. The form references HUD Handbooks 7417.1, 7475.13, and 7485.1 and .3 as supporting guidance documents.
