About this file

This document is an Attachment D Cost Response form for a PCA (Personal Care Assistant) Identification Card contract. The file outlines the payment structure for a Card Contractor responsible for producing and managing identification cards, with compensation based on the volume of cards distributed monthly. The pricing model allows for prorated payments if operations begin or end mid-month, with payment calculated by multiplying the actual card volume by the per-card price and extrapolating to a 30-day month.

The cost response section requires bidders to specify the price per 1,000 identification cards and include any applicable volume discounts. The contract covers comprehensive services including card production, request processing, photograph acceptance, technical assistance, card replacement, and multilingual support. The payment terms stipulate that the monthly payment covers all services rendered by the vendor for that month, with the specific pricing to be determined by the contractor's submitted rate per 1,000 identification cards.

Attachment D: Cost Response

The Card Contractor will be paid the appropriate price per PCA Identification Card distributed times the actual PCA Identification Card volume for each month. Should operations commence or terminate in the middle of a month, PCA Identification Card volume will be extrapolated to a 30-day month and the Card Contractor will be paid their actual volume times the extrapolated PCA Identification Card volume price per card.

For the avoidance of doubt, and except as provided in the last paragraph of Section 11.1 of the SOW, such monthly payment constitutes payment in full for all services under this SOW rendered that month by the Vendor. Such services include, but are not limited to:

· Process for producing Identification Cards that meets the requires of the SOW; and

· A method for receipt of requests, acceptance of photographs, provision of technical and informational assistance related to questions regarding the PCA Identification Card request process, replacement of lost PCA Identification Cards, and ability to provide multiple language options for such method.

Please indicate cost per 1,000 Identification Cards. Please include any volume discounts that may apply.
