About this file
This is a Past Performance Questionnaire template (Attachment D) for offerors responding to a Bureau of Engraving and Printing (BEP) solicitation for Consolidated Inks. The questionnaire requires offerors to submit at least three past performance references from contracts within the last five years that are similar in size, scope, and complexity to BEP's requirements.
The questionnaire includes sections for contract information, assessor details, and performance evaluations across multiple categories including business relations, timeliness, quality, and cost. Assessors, not offerors, must complete and submit the questionnaire directly to charles.underwood@bep.gov. Performance ratings range from Exceptional to Unsatisfactory, with detailed evaluation criteria provided for each rating level. The questionnaire specifically evaluates performance related to Letterpress Inks and requires assessors to indicate whether they would award another contract to the contractor.
