About this file
This is a Mandatory Site Visit Validation Form for a Request for Bids (RFB 758 2500000657) issued by the Commonwealth of Kentucky for services at the Robert E. Spiller Bowling Green Veterans Center located at 1111 Veterans Way in Bowling Green, Kentucky. Bidders are required to inspect the site where services are to be performed and must complete a mandatory site visit to satisfy themselves regarding all general and local conditions that may affect contract performance costs. To arrange the site visit, bidders must contact Will Humphrey at Will.Humphrey@ky.gov or (270) 438-9671. The completed and signed Site Visit Validation Form must be submitted with the bidder's response to be considered for award. Failure to tour the grounds and obtain the required signature from an authorized agency representative shall result in disqualification of the bid response, and the bidder assumes responsibility for verifying the actual size of areas specified in the solicitation.
The form explicitly prohibits disclosure of any pricing information to agency representatives during the site visit, and failure to comply with this requirement renders the bid non-responsive. The authorized RESBGVC representative and the company representative touring the area must both sign and date the validation form, certifying that all facility areas mentioned in the bid have been toured and that no pricing information was provided. A failure to inspect the site does not constitute grounds for claims after contract award. This document must be detached from the solicitation packet and taken directly to the agency representative for signature before submission with the bid response.
