About this file

This document is a Past Performance Questionnaire for Solicitation 36S79725R0001 issued by the VA National Acquisition Center Commodities and Services Acquisition Service. The form is designed to collect detailed performance feedback about a contractor, with six evaluation areas rated on an adjectival scale from Excellent (E) to Poor (P), including contract requirements met, schedule adherence, responsiveness to issues, effectiveness with unusual needs, and overall performance satisfaction. The questionnaire includes rating definitions ranging from "Excellent" (performance greatly exceeded requirements) to "Poor" (performance consistently failed to meet contract requirements), and requires the evaluator to provide comments for each rated category. The completed form should be mailed directly to Michelle Williams at the specified email address, with a note that contractors may be advised of adverse remarks while maintaining confidentiality of the evaluator's identity.
