About this file

This document is a WIC (Women, Infants, and Children) Infant Formula Bid Sheet Template designed for state agencies to conduct a single-supplier competitive bid solicitation for infant formula, aligned with federal requirements under 7 C.F.R. 246.10(e)(9)(Table 1) and 246.16a(c)(2, 4-7). The bid sheet covers three physical formula types (powdered, liquid concentrate, and ready-to-feed) across three infant age categories (0-3 months, 4-5 months, and 6-11 months), with estimated monthly distribution quantities ranging from 7 to 14,431 units depending on infant age and formula type.

The bid sheet requires manufacturers to provide detailed pricing information, including lowest wholesale full truckload prices, rebate bids per unit, and net prices, with specific certification requirements ensuring compliance with the Food, Drug and Cosmetic Act. Manufacturers must be able to supply 100% of the WIC Program's infant formula needs across all geographic areas outlined in the solicitation. The template emphasizes that no quantity guarantees are provided, and the estimated infant numbers are approximations excluding exclusively breastfed infants or those requiring exempt infant formulas. Bidders are expected to complete specific gray-highlighted cells with precise pricing data and sign the certification statements.
