About this file

This document is an Attachment D for Additional Terms and Conditions to an Enterprise Contract for Miscellaneous Property Insurance, issued by the Florida Department of Management Services (under Solicitation No. 25-64121509-ITB). The contract establishes terms for a State Term Contract for insurance services, with an initial term beginning on the date specified in the contract. The document indicates that upon written agreement, the Department and the Contractor may renew the Term Contract in whole or in part, in accordance with Florida Statute 287.057(13).

The document modifies standard contract terms by removing several sections from the original Form PUR 1000 and Enterprise Standard Terms and Conditions, including provisions related to transportation, delivery, packaging, installation, and cooperative purchasing. Key definitions are provided, specifically defining "Customer" as the agency or eligible user included on the insurance policy, and clarifying that customers other than the Department cannot modify contract or policy terms. The contract is competitively procured pursuant to Florida Statutes and is intended for use by state agencies and eligible users as defined in Florida Administrative Code.
