About this file

This is a Required Affidavit for Resident Bidder Status document associated with Solicitation/Contract #RFB 758 1900000475, a procurement opportunity involving the Commonwealth of Kentucky. The affidavit establishes eligibility criteria that bidders, offerors, and contractors must meet to claim resident bidder status under Kentucky Revised Statutes 45A.494(2). To qualify, an entity must be authorized to transact business in Kentucky on the date the contract is first advertised and must demonstrate continuous business presence through specific financial and compliance documentation requirements.

Bidders claiming resident bidder status must provide evidence that, for one year prior to and through the date of advertisement, they filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund, and maintained an active Kentucky workers' compensation policy. The bidding agency reserves the right to request supporting documentation to verify resident bidder status claims, and failure to provide such documentation upon request will result in bidder disqualification or contract termination. The affidavit requires notarization and must be executed under penalty of perjury by an authorized representative of the bidding entity.
