About this file
This document is a Proposer Information Certification Sheet for RFP# S-DASOBO-00015289 issued by the State of Oregon for GPS Satellite Tracking Collars and Associated Equipment for Various Species. The certification sheet functions as a required submission form that proposers must complete and sign to confirm their understanding of the RFP requirements, acceptance of the Price Agreement terms and conditions outlined in Attachment A, and acknowledgment of all RFP addenda. The firm offer is valid for 180 days following the closing date, and if awarded, the proposer agrees to perform the scope of work and meet performance standards as defined in the final negotiated Price Agreement. The document does not specify response dates, due dates, site visits, bidders' meetings, award dates, or contract terms in the certification sheet itself, as these details would be contained in the main RFP document.
The certification sheet requires proposers to make extensive representations regarding tax compliance, non-discrimination in employment and subcontracting practices, sexual harassment prevention policies (required for contracts anticipated at $150,000 or more), wage payment compliance, employee wage discussion rights, broadband service provision compliance, and exclusion from federal sanctions lists. Proposers must certify the absence of conflicts of interest and affirm that all proposal contents are truthful, accurate, and prepared independently without collusion or fraud. The document emphasizes that false or fraudulent statements, misrepresentations, or material omissions may constitute false claims under the Oregon False Claims Act (ORS 180.750 to 180.785) and subject proposers to associated liabilities and penalties. No pricing information, set-asides, incumbent status, or funding details are specified in this certification form.
