About this file

This is a Past Performance Questionnaire (PPQ) template for the FDA Nationwide Ergonomic Services RFQ No. 75F40126Q00019. The questionnaire is designed to be completed by government personnel who have direct experience with contractor performance on previous or current contracts. Responses must be submitted to Contract Specialist Melinda Kim at Melinda.Kim@fda.hhs.gov and copied to Contracting Officer Nicholas Bisher by February 13, 2026, at 2:00 PM Eastern Time. The form collects comprehensive performance data across five evaluation categories: Business Relations, Quality of Service, Cost Control, Timeliness of Performance, and Customer Satisfaction, with ratings assigned on a scale of 1 (Poor) to 4 (Excellent), or N/A if insufficient knowledge exists.

The questionnaire template captures contract-specific information including contractor name, contract number, performance period, contract value, contract type, and subcontractor details. Each evaluation section includes multiple performance criteria with numeric rating scales supported by detailed rating guidelines. The Business Relations section assesses contractor personnel qualifications, management effectiveness, responsiveness, and flexibility. Quality of Service evaluates compliance, technical responsiveness, report accuracy, and technical excellence. Cost Control examines cost forecasting, billing accuracy, and cost efficiencies. Timeliness of Performance measures milestone achievement, reliability, and schedule compliance. The Customer Satisfaction section concludes with yes/no questions regarding contractor recommendation for award, likelihood of rehire, and value assessment relative to cost, with space for detailed commentary supporting all ratings.
