About this file

This is a Past Performance Questionnaire template used by U.S. Customs and Border Protection (CBP) to evaluate contractor capability and capacity for pending solicitations. The questionnaire is divided into three sections: Section A collects basic contractor/offeror information including company name, division, address, and NAICS code; Section B gathers contract details such as period of performance, contract number, contracting office, performance location, contracting officer contact information, contract dates, awarded and current dollar values, competitive status, contract type (FFP, FPI, FPR, CPFF, CPIF, CPAF, or other), key subcontractors with DUNS and CAGE codes, and small business utilization information including subcontracting plan details and Individual Subcontracting Report (ISR) or Summary Subcontracting Report (SSR) submission dates.

Section C contains the Past Performance Survey to be completed by the offeror's reference and submitted directly to richard.m.kirby@cbp.dhs.gov no later than Mid-March 2026 at 9:00 AM Eastern Time. This section evaluates contractor performance across eight criteria using a six-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Unknown/N/A): Quality of Service, Schedule Control, Cost Control, Federal Information Security Controls (including FISMA compliance and Authorization to Operate), Management of Key Personnel, Utilization of Small Business Concerns, Adapting to Change, and Transition-in/Transition-out activities. Each rating requires supporting narrative comments. The survey concludes with a final recommendation section where the assessing official indicates whether they would "Definitely Would," "Probably Would," "Probably Would Not," or "Definitely Would Not" award to the contractor, along with the assessing official's contact information and signature.
