About this file
This is a Past Performance Questionnaire template for a Request for Quote (RFQ 20156753) issued by the Department of Homeland Security Customs and Border Protection (CBP) for electronic fingerprinting services. The questionnaire is designed to be completed by quoters (Section A and B) and their references (Section C), with references required to submit their completed assessments directly to richard.m.kirby@cbp.dhs.gov no later than April 4, 2026 at 12:00 PM Eastern Time.
The questionnaire requires contractors to provide company information, contract/project details from the past three years, and contract specifications including period of performance, contract number, contracting office, performance location, award and completion dates, dollar values, contract type (FFP, CPFF, CPIF, CPAF, or other), and small business utilization information. References are asked to evaluate contractor performance across eight key criteria: Quality of Service, Schedule Control, Cost Control, Federal Information Security Controls (FISMA/ATO compliance), Management of Key Personnel, Utilization of Small Business Concerns, Adapting to Change, and Transition-in/Transition-out capabilities. Each criterion uses a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Unknown/N/A) with space for narrative explanations. The questionnaire concludes with a recommendation section asking the assessing official whether they would definitely, probably, or would not award future work to the contractor based on past performance.
