About this file

This document is an asbestos analysis report from Atlas Laboratories for the Oregon Department of Forestry, focusing on material testing for a project located at 801 Gales Creek Rd. in Forest Grove, Oregon. The laboratory analyzed multiple samples of window putty and building wrap from different locations (FGWH and FGFC), using Polarized Light Microscopy to detect asbestos content. The analysis was completed on 06/23/2025 with a 2-day turnaround time, identifying chrysotile asbestos at 2% concentration in three specific window putty samples (one from FGWH SE and two from FGFC NE and NW locations).

The report includes detailed documentation of sample locations, materials tested, and asbestos percentages, with most samples showing no asbestos presence. The chain of custody form was completed by a representative from the Oregon Department of Forestry, indicating a systematic approach to material testing. Atlas Laboratories provided a comprehensive disclaimer noting their liability is limited to the cost of analysis and that the report relates only to the specific samples tested, with interpretation and use of test results being the client's responsibility.

Atlas Laboratories maintains liability to cost of analysis. This report relates only to the samples reported and may not be reproduced, except in full without written permission by Atlas. Atlas bears no responsibility for sample collection activities or analytical method limitations. Interpretation and use of test results are the responsibility of the client. This report must not be used by the client to claim product certification, approval or endorsement by NVLAP, NIST, NIOSH or any other agency of the federal government. Non-friable organically bound materials present a problem matrix and therefore Atlas recommends gravimetric reduction prior to analysis. Samples received in good condition unless otherwise noted. Transmission Electron Microscopy asbestos identification and lead paint analysis will be available and performed by laboratories by proxy. Original analysis documents are available upon request of the client.
