About this file
This file is a Required Affidavit for Bidders, Offerors and Contractors mandated by the Commonwealth of Kentucky under KRS 45A.110 and 45A.115, applicable to the Chemical Analysis Testing Services contract opportunity in Kentucky. The affidavit must be completed by an authorized representative of the contracting party, notarized, and submitted to the Commonwealth prior to contract award or renewal. The attestation confirms that the contractor has not knowingly violated Kentucky's campaign finance laws and that the contract award will not violate any provision of those laws. The affidavit is effective for one year from the date of execution, and the Commonwealth retains the right to request an updated affidavit at any time during the contract term.
The document requires the contractor to provide company name, address, Commonwealth of Kentucky vendor code if available, and the authorized representative's signature, printed name, title, and date. A notary public must witness and certify the affidavit, providing their commission expiration date and the state and county where the notarization occurs. The term "knowingly" is defined to mean that the bidder or offeror is aware or should have been aware of the existence of a violation, establishing a standard of reasonable knowledge rather than actual knowledge alone.
