About this file

This is an affidavit document required by the Commonwealth of Kentucky for bidders, offerors, and contractors participating in state contracts, including the Kentucky 811 Training Module Re-Bid opportunity. The document outlines three required affidavits that must be completed and notarized by authorized representatives of contracting entities. The primary affidavit requires contractors to attest under penalty of perjury that they have not knowingly violated Kentucky campaign finance laws and that contract award will not violate any campaign finance provisions. This affidavit remains effective for one year from the date of execution, though the Commonwealth reserves the right to request updated affidavits at any time.

The document includes two additional affidavits for entities claiming resident bidder status or qualified bidder status. Resident bidder affidavits require entities to confirm they are authorized to conduct business in Kentucky and have maintained Kentucky corporate income tax filings, unemployment insurance fund payments, and workers' compensation policies for at least one year prior to contract advertisement. Qualified bidder affidavits apply to nonprofit corporations furthering Kentucky statutes or qualified nonprofit agencies serving individuals with severe disabilities, which must employ such individuals for at least seventy-five percent of direct labor hours on listed products or services. The bidding agency reserves the right to request supporting documentation for either status claim, with failure to provide documentation resulting in bidder disqualification or contract termination. All affidavits must be executed before a notary public and submitted to the Commonwealth as a condition of contract award.
