About this file
This document is a Vendor Disclosure Statement for the Colorado Department of Corrections (CDOC) related to Solicitation Number DQ 2024000229, specifically for a commercial washer and dryer procurement at the Limon Correctional Facility. The form requires vendors to disclose details about where contract services will be performed, with particular emphasis on identifying any work conducted outside of the United States or Colorado. The disclosure is mandated by Colorado Revised Statutes §24-102-206 and is a prerequisite for contract solicitation, though certain types of contracts (such as Medicare, medical assistance, and construction contracts) are exempt from this requirement.
The form provides a template for prospective vendors to transparently report the geographic locations of contract and subcontract work, with sections for listing counties and states where services will be performed. Vendors must explain any anticipated work outside the United States or Colorado, detailing why such external performance is necessary or advantageous. The document requires a signature, printed name, and title from the vendor, signaling an official commitment to disclosure and transparency in the state contracting process. While specific details about the washer and dryer installation are not included in this disclosure statement, it serves as an administrative document to ensure vendor accountability and geographic work transparency for the CDOC procurement.
