About this file
Attachment B is a State Corporation Commission compliance certification form for RFP #2026-FA01 issued by Culpeper County, Virginia. The form requires all offerors to provide documentation of their State Corporation Commission identification number or explain their exemption from Virginia business authorization requirements pursuant to Virginia Code 2.2-4311.2. Offerors must select one of three options: provide their SCC identification number if organized or authorized to transact business in Virginia; provide their SCC identification number and state of incorporation if operating as a foreign business entity authorized in Virginia; or provide a written explanation if not required to obtain SCC authorization. The certification requires the firm's authorized representative to sign and attest that all information is accurate and that the offeror will maintain compliance throughout the contract term, with notice required to the County of any changes in SCC registration status during performance.
This compliance form is a mandatory submission requirement for all proposals in response to the forensic financial review and auditing services RFP. Failure to include proper SCC documentation or certification may result in proposal rejection. The form emphasizes that Culpeper County may void any contract if the awarded contractor fails to maintain required SCC compliance during the contract period. All offerors must complete the applicable certification section with firm name, authorized signature, printed name, title, and date of submission.
