About this file
This document is an Attachment B - Response Form for a Request for Responses (RFR) issued by the Commonwealth of Massachusetts Executive Office of Health and Human Services (EOHHS) for Clinical and Safety Risk Assessment (CSRA) Sites, Staffing, and Related Services. The RFR seeks to procure qualified vendors to operate and staff one or more CSRA Sites, which will provide basic clinical assessments and support to homeless families and individuals, including those with medical vulnerabilities. Bidders must submit a cover letter, organizational information, details on any proposed subcontractors, financial statements, a description of relevant experience, an implementation plan, and a staffing plan. Bids are due on March 1, 2024. The resulting contract(s) will have an initial term of 2 years, with the possibility of up to 3 additional 1-year renewals.
The RFR includes a completed budget template (Attachment C) that bidders must submit, along with a budget narrative. The RFR does not specify any set-asides or preferred pricing terms. EOHHS will evaluate bids to determine the most qualified vendor(s) to provide the requested services. Funding for the contract(s) will come from EOHHS, the Executive Office of Housing and Economic Development (EOHED), and the Office of Refugees and Immigrants (ORI).
