About this file

This is a Questionnaire form used by the Florida Department of Financial Services, Office of Insurance Regulation, to evaluate qualified vendors seeking to provide financial and market conduct examination services for authorized insurers, health maintenance organizations, prepaid limited health service organizations, and discount plan organizations. The questionnaire assesses respondents' examination capabilities across multiple service types including full scope and limited scope risk-focused financial examinations, market conduct examinations, actuarial services, information technology reviews, administrative supervision, investment analysis, and reinsurance examination. Qualified respondents must demonstrate examiner availability, professional certifications, training completion, prior examination experience, and adherence to NAIC Financial Condition Examiners Handbook and NAIC Market Regulation Handbook standards. The solicitation requires respondents to disclose potential conflicts of interest, including family members employed by the Florida Office of Insurance Regulation and recent former employees of the Office.

The questionnaire requires submission of standard hourly rates for various examination positions including Examination Manager, Examiner-in-Charge, Participating Examiner, Head Actuary, Participating Actuary, Cyber Security/IT Forensics Specialist, Head IT Specialist, Participating IT Specialist, Investment Specialist, Reinsurance Specialist, and legal services, with separate rate schedules for years one through three of the contract term. Respondents must certify compliance with Florida Statutes requirements including non-discriminatory vendor status, drug-free workplace implementation, E-Verify system utilization, and Florida Department of State registration. Supporting documentation required includes proof of registration, resumes of examination team members, NAIC training completion documentation, and disclosure of any litigation or regulatory suspensions. The questionnaire includes provisions for identical tie response considerations, prioritizing certified minority-owned firms, veteran-owned businesses, Florida-based businesses, and drug-free workplace certified companies. Respondents must acknowledge receipt of Office notices regarding examination assignment procedures, invoice submission requirements, NAIC Handbook compliance, and new Practical Training and Talent Development requirements for Office examination staff participation.
