About this file
This is a Past Performance Questionnaire form for evaluating contractor performance on federal contracts related to the Health Care for Homeless Veterans (HCHV) program. The form is specific to Solicitation Number 36C24426Q0925 for Butler VAMC in Pennsylvania and is designed to be completed by government evaluators and submitted to Enrico.Tan1@va.gov.
The questionnaire requires evaluators to document contractor identification information (including contract number, period of performance, and negotiated pricing), provide evaluator credentials and tenure, and assess contractor performance across three primary elements: Quality of Services/Care, Qualified Personnel, and Business Relations. For each element, evaluators must provide detailed narratives addressing specific criteria such as the contractor's ability to provide therapeutic and rehabilitative services in a safe environment, deliver qualified management and supervisory personnel, and demonstrate willingness to correct noncompliance issues and maintain effective working relationships. Evaluators assign adjectival ratings (Excellent, Good, Satisfactory, Marginal, or Unsatisfactory) based on defined performance descriptors, with mandatory narrative explanations for Marginal or Unsatisfactory ratings. The form also requires evaluators to identify overall contractor strengths and weaknesses, indicate whether they would award to the contractor again, and provide an overall Past/Present Performance rating with supporting comments.
