About this file
This is a Cost Proposal Form for RFP 758-20000000265 issued by the Commonwealth of Kentucky for the implementation of a Point of Sale (POS) System and Customer Relationship Management (CRM) system. The project is being conducted for KHS (Kentucky Health Services) and requires vendors to provide comprehensive pricing for a multi-year implementation and hosting arrangement. The proposal encompasses a nine-year contract term, with Year 1 covering all implementation costs including configuration, software, Blackbaud system interface, project management, licensing, hosting, and maintenance support. Years 2 through 9 cover only hosting and maintenance costs for up to 75 users. Training costs for go-live are excluded from Year 1 implementation costs and are provided separately on a fully loaded hourly rate basis.
Vendors must provide detailed cost breakdowns including configuration and implementation fees, annual hosting and maintenance costs for each contract year, custom development hourly rates for software architects, developers, business analysts, and training personnel, and POS hardware costs for three specific areas: CKH Front Desk, KMHM Admissions, and Gift Shop/Retail Operations. Additional pricing is required for monthly software licensing of hard-wired and mobile POS terminals. The Commonwealth of Kentucky is exempt from sales tax, and all hosting and maintenance costs must include security compliance, software updates, help desk support, bug fixes, and licensing fees. Vendors must also identify supplemental POS system products available through existing Commonwealth Master Agreements for direct state purchase.
