About this file

This is a Conflict of Interest Questionnaire Form attachment for the City of Leander's solicitation for fully insured Medical, Dental, and Vision Benefits. The City of Leander is requesting proposals from a single vendor to provide comprehensive health insurance coverage for approximately 410 employees with proposed coverage for the benefit period January 1, 2027 through December 31, 2027. Proposals are due August 21, 2026 at 2:00 p.m. CST, with a questions deadline of August 6, 2026. Interviews, if necessary, are scheduled for August 31 and September 1, 2026. The initial contract term is twelve months from award, with options for up to four one-year renewals. The vendor must be experienced in providing medical, dental, and vision plans to Texas municipalities and must maintain Professional E&O Liability insurance of at least $1,000,000.

The City explicitly is not requesting consultant, agent, or broker services, as HUB International serves as the City's consultant and agent of record. Medical commissions are net zero percent, dental commissions are zero percent, and voluntary vision commissions are 15 percent. Current medical rates range from $455.87 to $1,872.59 by plan type and coverage level, dental rates from $10.65 to $98.62, and vision rates from $8.08 to $23.76. Vendors must provide fully insured plans with takeover credits for deductibles, coinsurance, and waiting periods satisfied prior to the effective date, and medical premium proposals must exclude premium taxes per Texas Insurance Code Chapter 222, Section 222.002. Proposals will be evaluated on cost (40 points), claims processing and management (40 points), integrated systems/technology initiative (10 points), and employee systems and engagement (10 points). Respondents must submit a Conflict of Interest Questionnaire, a Subcontractor Information Form, and references from at least three comparable municipal or government agencies within the last three years, with failure to provide required references or negative reference responses resulting in automatic disqualification.
