About this file

This is a bid form and instructions document from the Maryland State Department of Education, Division of Rehabilitation Services (DORS) for a Home Accessibility Modifications project designated as CS# 26-18. The procurement is a Certified Small Business (CSB) designated contract for home accessibility modifications and residential construction services for a DORS client. Bidders must provide pricing for multiple construction divisions including General, Existing Conditions, Woods and Plastics (Rough Carpentry), Doors and Windows (Automatic Door Opener), Conveying System (Curved Indoor Stairlift), and Electrical work. The bid form requires vendors to submit separate pricing for materials and labor for most divisions, with Division 01 requiring only one price. Bidders must complete a timeline section indicating the number of weeks or days required for pre-construction activities, construction commencement, and overall project completion, with unrealistic timelines subject to bid disqualification.

All pricing must be entered in designated blue spaces within the Excel workbook, with automatic calculation of total bid price. Vendors must provide a two-year warranty covering all components, maintenance, and repairs in addition to manufacturers' warranties. The bid form must be signed by an authorized representative and include company information, Federal ID number, and eMM number. Invoicing will be processed through a dedicated portal with specific requirements for partial invoices (requiring paid manufacturer receipts for equipment over $5,000) and final invoices upon delivery of the modified vehicle. Final invoices must include itemized equipment details with manufacturer and serial numbers, warranty documentation, and consumer-signed documents. All inquiries regarding specifications and drawings should be directed to Brittany Owens at MSDE procurement via email, while workbook technical issues should be addressed to Pam Post.
