About this file
This file is an attachment containing required affidavits for bidders, offerors, and contractors seeking to contract with the Commonwealth of Kentucky for Golf Course Chemicals. The document consists of three separate affidavit forms that bidding entities must complete and submit as part of the procurement process. All affidavits must be notarized and are effective for one year from the date of execution. The first affidavit requires contractors to attest under penalty of perjury that they have not knowingly violated Kentucky campaign finance laws and that contract award will not violate any provisions of state campaign finance statutes. The second affidavit is for entities claiming resident bidder status, requiring attestation that the bidder is authorized to transact business in Kentucky and has maintained Kentucky corporate income tax filings, unemployment insurance fund payments, and workers' compensation coverage for at least one year prior to and through the contract advertisement date. The third affidavit applies to entities claiming qualified bidder status, specifically nonprofit corporations or qualified nonprofit agencies for individuals with severe disabilities operating under Kentucky Revised Statutes Chapter 163 and KRS 45A.465(3).
The bidding agency retains the right to request documentation supporting any claimed status at any time, with failure to provide such documentation resulting in bidder disqualification or contract termination. Qualified nonprofit agencies must employ individuals with severe disabilities for at least seventy-five percent of direct labor hours in the manufacture or provision of products or services and must be registered and in good standing with the Kentucky Secretary of State. All affidavits require authorized representative signature, printed name, title, and date, along with company information and notarization details including notary seal and commission expiration date.
