About this file
This is an affidavit document issued by the Commonwealth of Kentucky requiring bidders, offerors, and contractors to certify compliance with state procurement regulations. The document contains three separate required affidavits that must be executed and notarized before contract award or renewal. Each affidavit remains effective for one year from the date of execution. The affidavits address campaign finance law compliance, resident bidder status requirements, and qualified bidder status claims under Kentucky Revised Statutes and Kentucky Administrative Regulations.
The first affidavit requires contractors to attest under penalty of perjury that they have not knowingly violated Kentucky campaign finance laws and that contract award will not violate any campaign finance provisions. The second affidavit applies to bidders claiming resident bidder status and requires attestation that the entity is authorized to transact business in Kentucky and has maintained Kentucky corporate income tax filings, unemployment insurance fund payments, and workers' compensation coverage for the year prior to bid advertisement. The third affidavit pertains to bidders claiming qualified bidder status, including nonprofit corporations advancing purposes under Kentucky Chapter 163 or qualified nonprofit agencies employing individuals with severe disabilities for at least seventy-five percent of direct labor hours. All affidavits must be completed by authorized representatives, notarized, and submitted to the Commonwealth. The bidding agency reserves the right to request supporting documentation and may disqualify bidders or terminate contracts for failure to provide such documentation upon request.
