About this file
This document is a Required Affidavit for Bidders, Offerors, and Contractors issued by the Commonwealth of Kentucky, specifically related to Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. The affidavit is designed to be effective for one year from its execution date and requires bidders, offerors, or contractors to submit a sworn statement confirming they have not knowingly violated campaign finance laws of Kentucky. The document must be completed by an authorized representative of the contracting party, notarized, and returned to the Commonwealth.
The affidavit requires the contractor to attest under penalty of perjury that they are aware of and have not violated campaign finance laws, with the understanding that "knowingly" means being aware or should have been aware of any potential violations. The Commonwealth retains the right to request an updated affidavit at any time. The form includes spaces for the bidder or offeror's name, address, vendor code, signature, printed name, title, date, and notary public details. This document serves as a legal mechanism to ensure transparency and compliance with Kentucky's campaign finance regulations for potential contractors.
