About this file

This is a State Corporation Commission Form attachment for IFB# 154:26-115641 related to a janitorial services contract for nine stand-alone Virginia Department of Motor Vehicles Customer Service Center locations. The form establishes eligibility and compliance requirements for bidders participating in this procurement opportunity.

The form requires bidders to certify their business status and Virginia presence through one of four options: holding a current State Corporation Commission identification number as a registered business entity; confirming they are not a corporation, limited liability company, limited partnership, registered limited liability partnership, or business trust; demonstrating status as an out-of-state business entity without regular employees, agents, offices, facilities, or inventories in Virginia; or providing legal counsel opinion documenting out-of-state status and explaining why current Virginia contacts do not constitute transacting business in the Commonwealth. Additionally, bidders with pending SCC applications may request a waiver to submit their identification number after the bid deadline, subject to the Commonwealth's sole discretion in granting such relief.
