About this file

This is a Statement of Qualifications cover sheet issued by the Mississippi State Department of Health for an External Program Evaluator position supporting the CDC-funded WISEWOMAN (Well-Integrated Screening and Evaluation for Women Across the Nation) program (RFQ 1301-27-R-RFQF-00003). The WISEWOMAN program aims to reduce cardiovascular disease risk among eligible, low-income, underinsured or uninsured, underserved women by integrating clinical screening with evidence-based interventions including self-measured blood pressure monitoring, lab work, lifestyle change programs, and tobacco cessation services. Qualifications responses must be submitted no later than September 28, 2026, at 2:00 p.m. CST in sealed bid format. The contract term runs for 2.5 years from December 1, 2026, through November 30, 2028, with renewal options available for up to three additional years, not to exceed a cumulative five-year term. No pre-submittal conference or site visit will be held, and the anticipated notice of intent to award is September 30, 2026.

The contract is structured as a firm-fixed price agreement at $50,000 annually for a total of $125,000 over the initial 2.5-year period, with payment distributed as 20 percent upon completion of the Evaluation Plan, 40 percent prorated across quarterly data briefs, and 40 percent upon completion of the annual report and dashboard. Qualified evaluators must hold a Master's degree or higher in Public Health, Epidemiology, Biostatistics, or a related field, with a minimum of 10 years of applied public health program evaluation experience, including expertise in CDC-funded cooperative agreements and cardiovascular risk reduction interventions. Bidders must be currently registered in the System for Award Management (SAM), provide at least three professional references (with agency staff contacting at least two within two business days of the opening date), and demonstrate financial stability through audited financial statements. Evaluation will occur using a 100-point scale across three phases: Compliance (Pass/Fail), Technical (35 points), and Management (30 points). Core evaluation activities may not be subcontracted without prior written approval, and all program data remains the property of the Mississippi State Department of Health, with any publications requiring written permission and collaborative authorship from MSDH leadership.
