About this file
This Quality Assurance Surveillance Plan (QASP) outlines surveillance methods for water testing and maintenance services at the Kansas City VA Medical Center. The document details six primary surveillance methods including direct observation, periodic inspection, random sampling, performance metrics review, contractor self-reporting, and customer feedback. Surveillance frequencies vary by performance area, ranging from weekly (water sampling, water management program data entry) to quarterly (laboratory certification, contractor performance review).
Performance metrics will evaluate the contractor on completion rates of scheduled maintenance, test result accuracy, calibration schedule compliance, documentation quality, emergency response time, system reliability, and quality control findings. Key stakeholders include the Contracting Officer (CO), who maintains contractual authority, the Contracting Officer's Representative (COR), who conducts surveillance and documents findings, and Engineering Service/Infection Control, which provides technical oversight and compliance verification. The QASP will be signed by the contractor's Program Manager, COR, and Administrative Contracting Officer after contract award.
