About this file
This is a Proposer Information and Certification Sheet for RFP C2026132 issued by the Eastern Oregon Border Economic Development Board for Third Party Administrative Services. The document serves as the required certification form that prospective proposers must complete and submit as part of their proposal response. The RFP requires proposers to provide their legal name, business address, state of incorporation, entity type, and primary contact information. Proposals submitted in response to this RFP constitute firm offers valid for 180 days following the closing date. Upon contract award, the selected proposer must perform the scope of work and meet performance standards as defined in the final negotiated contract documents, which are referenced in Attachment F of the RFP.
The certification sheet requires proposers to affirm compliance with multiple statutory and regulatory requirements, including Oregon tax law compliance, non-discrimination in employment and subcontracting practices, wage and hour compliance under ORS 652.220, and sexual harassment prevention policies for contracts with an anticipated price of $150,000 or more. Proposers must also certify that neither they nor their employees, agents, or subcontractors appear on the Office of Foreign Assets Control's Specially Designated Nationals and Blocked Persons list or the System for Award Management exclusions lists. Additionally, proposers must disclose any actual or potential conflicts of interest and certify that all proposal contents are truthful, accurate, and prepared independently without collusion or fraud, understanding that false or fraudulent statements may constitute claims under the Oregon False Claims Act.
