About this file

This file is a Service Provider Security Assessment Questionnaire required for a Watercraft Decal Fulfillment contract opportunity with the State of South Carolina. The questionnaire serves as a mandatory security compliance document that potential service providers must complete and submit with their bids. The assessment covers eleven core security requirements and requires bidders to provide detailed information regarding their information security practices, policies, and certifications. No specific response dates, due dates, site visits, bidder meetings, award dates, or contract terms are identified within this questionnaire document itself.

The questionnaire requires service providers to address critical security elements including access controls, disaster recovery and business continuity plans, employee and contractor vetting procedures, third-party contractor management, industry certifications (such as ISO/IEC 27001 or AICPA SOC 2), physical security measures, data encryption protocols, breach detection controls, incident response procedures, and post-contract data management and destruction protocols. Additionally, bidders must identify any third parties with access to government information and commit to maintaining audit logs and security certifications throughout the contract duration. An authorized company representative must sign the questionnaire attesting to the accuracy of all provided information. The questionnaire explicitly states that responses should reference supporting documentation and must be read in conjunction with related service provider security clauses and representations.
