About this file

This document is a Debarment, Suspension, Ineligibility and Voluntary Exclusion Certification form required by the City of Long Beach for vendors and contractors who may receive federal grant funds through their business relationship with the City. The certification complies with Executive Order No. 12549 and Title 2 CFR Part 180 Subpart C. Vendors must certify under penalty of perjury that neither their business entity nor any principals (including owners, directors, partners, officers, managers, or persons with substantial influence in covered transactions) are currently under suspension, debarment, voluntary exclusion, or determination of ineligibility by any federal department or agency. Additionally, vendors must confirm they have not been suspended, debarred, voluntarily excluded, or declared ineligible within the three-year period preceding the bid or agreement, do not have a proposed debarment proceeding pending, have not been indicted or convicted of fraud or official misconduct within three years, and have not had public transactions terminated for cause or default within three years. The City of Long Beach checks the System for Award Management at www.sam.gov to verify that awarded contractors and consultants are not debarred or suspended.

Vendors must complete and sign the certification form with the authorized representative's name, title, signature, and date before submitting their bid, proposal, or agreement. If any reorganization, management turnover, or change of principals' status occurs, written notice must be submitted within 21 days. Any exceptions to the certification must be attached with an explanation, indicating to whom the exception applies, the initiating agency, and dates of action. Exceptions will not necessarily result in denial of award but will be considered in determining bidder responsibility. Prospective consultants should perform their own search on www.sam.gov for their company and associated persons. Providing false information may result in criminal prosecution or administrative sanctions. For questions regarding form completion, vendors should contact the Purchasing Division in the City of Long Beach Business Relations Bureau at 562-570-6200.
