Attachment A 36C24825Q0595
(i) In accordance with FAR Part 13, this combined synopsis/solicitation for commercial items is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation;
quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, North Florida/South Georgia Veterans Health System has a Requirement to procure: Fire Alarm Testing W/Minor Repair(s) as needed. The anticipated contract will be:
Base year with four (4) one (1) year option periods.
(ii) The combined synopsis solicitation number is 36C24825Q0361 It’s issued as a Request for Quotation (RFQ). The North American Industry Classification System (NAICS) code is 561621.
• A site-visit is required. (See Page 11)
(iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2025-03 (01-17-2025).
(iv) The combined synopsis solicitation is 100% Service-Disabled Veteran Owned Small Business (SDVOSB). The NAICS code is 561621. Interested SDVOSB’s shall be listed and registered in https://veterans.certify.sba.gov.
(v) This combined synopsis solicitation is for the procurement of Fire Alarm Testing W/Minor Repair(s) as needed. All equipment provided shall be new and functioning properly.
Responses shall be on an ALL or None bases, no partial submission. The Government reserves the right to make no award at all.
(vi) A description of the items to be acquired, shipped, delivered and/or installed in accordance with applicable Government need(s) as follows:

(viii) Provision 52.212-2, Evaluation – Commercial Products and Commercial Services (NOV 2021), applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price considered. Government will evaluate quotes and award to the offeror which represents best value based on: Technical, Delivery and Price.
Technical and Delivery when combined is more important than price.
• An evaluation will be conducted in accordance with FAR Part 13.106-2(b)(3) to determine the best value.
• Offerors shall submit a written Technical Response/Summary demonstrating their ability to complete all the requirements specified in the Statement of Work (SOW).
Offerors are highly encouraged to be concise, factual, and comprehensive to meet or exceed the Governments Fire Alarm System W/Minor Repairs requirement.
• Offerors shall submit Attachment C demonstrating their best pricing including any favored discounts and ability to me the projected delivery schedule provided in Attachment C- Price/Delivery Schedule.
• Provide applicable licenses and/or certifications as requested in accordance with Attachment B.

52.225-18 Place of Manufacture (AUG 2018).
(a) Definitions. As used in this clause— “Manufactured end product” means any end product in product and service codes (PSCs)
1000-9999, except—
(1) FPSC 5510, Lumber and Related Basic Wood Materials;
(2) Product or Service Group (PSG) 87, Agricultural Supplies;
(3) PSG 88, Live Animals;
(4) PSG 89, Subsistence;
(5) PSC 9410, Crude Grades of Plant Materials;
(6) PSC 9430, Miscellaneous Crude Animal Products, Inedible;
(7) PSC 9440, Miscellaneous Crude Agricultural and Forestry Products;
(8) PSC 9610, Ores;

“Place of manufacture” means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.
(b) For statistical purposes only, the offeror shall indicate whether the place of manufacture of the end products it expects to provide in response to this solicitation is predominantly—

852.212-71 Gray Market and Counterfeit Items (FEB 2023)
(a) No used, refurbished, or remanufactured supplies or equipment/parts shall be provided. This procurement is for new Original Equipment Manufacturer (OEM) items only.
No gray market items shall be provided. Gray market items are OEM goods intentionally or unintentionally sold outside an authorized sales territory or sold by non-authorized dealers in an authorized sales territory.
(b) No counterfeit supplies or equipment/parts shall be provided. Counterfeit items include unlawful or unauthorized reproductions, substitutions, or alterations that have been mismarked, misidentified, or otherwise misrepresented to be an authentic, unmodified item from the original manufacturer, or a source with the express written authority of the original manufacturer or current design activity, including an authorized aftermarket manufacturer.
Unlawful or unauthorized substitutions include used items represented as new, or the false identification of grade, serial number, lot number, date code, or performance characteristics.
(c) Vendor shall be an OEM, authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions.

Predecessor of Offeror (AUG 2020)
(a) Definitions. As used in this provision–
“Commercial and Government Entity (CAGE) code” means–
(1) An identifier assigned to entities located in the United States or its outlying areas by the
