About this file

This document is a Past Performance Questionnaire and Cover Letter issued by the 87th Contracting Squadron for evaluating contractor performance on a Control Tower Window Cleaning Services contract at Joint Base McGuire-Dix-Lakehurst, New Jersey.

The questionnaire is distributed to past customers or points of contact who have worked with firms bidding on the window cleaning contract. Respondents are required to complete sections documenting the offeror's information, respondent organization details, and specific contract information from prior engagements. The evaluation criteria assess performance across five categories: Performance and Supportability (quality service, personnel qualifications, deliverable completion, labor retention, and responsiveness to issues); Management (overall effectiveness, communication with government representatives, cooperation on solutions, subcontractor management, and quality control); Timeliness/Schedule (personnel recruitment speed, employee retention, replacement planning, credentialing package timely submission, and complaint resolution). Respondents rate each criterion on a scale of 1-5 (Exceptional, Satisfactory, Marginal, Unsatisfactory, or Neutral/Unknown). The questionnaire also includes yes/no questions regarding contract award likelihood, default termination status, and corrective action notices. All Unsatisfactory, Marginal, or Exceptional ratings require supporting comments; failure to provide comments renders the questionnaire void. Completed questionnaires must be submitted directly to Adam Swenson (adam.swenson.2@us.af.mil) and Adam Donofrio (adam.donofrio@us.af.mil), the Contracting Officer.
