About this file
This is a Past Performance Questionnaire (PPQ) template designed for evaluating contractor performance on federal procurements under FAR Part 15. The document is structured as an appendix to the Draft RFP for NESDIS Consolidated Antenna and Ground Systems Services (NCAGSS) and serves as a reference evaluation tool for assessing past performance of bidders on this opportunity.
The PPQ requires reference evaluators to complete four main sections: contract data identifying the project, contractor name, contract number, contract type, and complexity level; a performance evaluation assessing five key areas—quality of product or service, schedule/timeliness, business relations/management, cost control, and small business subcontracting—using a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory); evaluator comments addressing mission impact, sustainment innovation, and overall recommendation for future contracts; and evaluator identification information. The overall CPARS rating translates to a final Confidence Rating (Very High, High, Some, or Low Confidence) that the government will use to assess likelihood of successful performance. The questionnaire includes specific evaluation criteria for each performance area and requires supporting documentation for all ratings. Completed forms should be returned to the Contracting Officer at jame.chang@noaa.gov.

This Past Performance Questionnaire (PPQ) is structured for a federal procurement (e.g., FAR Part 15). It covers real-world program integration, operation, maintenance, sustainment, and requirements.
PAST PERFORMANCE QUESTIONNAIRE (PPQ)
Project Title:
Contractor Name:
SECTION I: CONTRACT DATA
(To be completed by the Reference Evaluator)
· Contract Number / Task Order: ________________________________________
· Contract Type (FFP, CPFF, etc.): ______________________________________
· Complexity: [ ] High [ ] Medium [ ] Low
· Key Scope Elements: (e.g., Program Management, Operations, Maintenance, and sustainment…)
SECTION II: PERFORMANCE EVALUATION
In the table below, indicate your rating for the contractor’s performance by placing an “X” in the appropriate block to the right of each question. Provide supporting information for each response in the space provided. Attach additional pages if more space is needed. The performance rating scale is defined as follows:
Code Performance Rating
E EXCEPTIONAL – Performance meets contractual requirements and exceeds many requirements to the Government’s benefit. The contractual performance being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
V VERY GOOD – Performance meets contractual requirements and exceeds some requirements to the Government’s benefit. The contractual performance being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
S SATISFACTORY – Performance meets contractual requirements. The contractual performance being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
| M | MARGINAL – Performance does not meet some contractual requirements. The contractual performance being assessed reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented. |
| U | UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance being assessed contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective. |
*Note to Evaluators: When completing this questionnaire, please evaluate each individual performance area using the standard CPARS adjectival rating scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory).
Once all individual evaluation areas have been scored, an Overall Performance Rating will be generated based on the culmination of these individual scores. This overall adjectival rating will then be directly equated to a final Confidence Rating, which the Government will use to assess the likelihood of successful performance on the current requirement.
Please refer to the matrix below to see how the Overall CPARS Rating translates into the Final Confidence Rating:
Overall CPARS Rating
Equivalent Confidence Rating
Description of Confidence Expectation
Exceptional
Very High Confidence
Based on the evaluated performance record, there is a high expectation that the offeror will successfully perform the required effort.
Very Good
High Confidence
Based on the evaluated performance record, there is a strong expectation that the offeror will successfully perform the required effort.
Satisfactory
Some Confidence
Based on the evaluated performance record, there is a reasonable expectation that the offeror will successfully perform the required effort.
Marginal
Low Confidence
Based on the evaluated performance record, there is a low expectation that the offeror will successfully perform the required effort.
Unsatisfactory
Low Confidence
Based on the evaluated performance record, there is no expectation that the offeror will successfully perform the required effort.
