About this file

This document contains a past and present performance questionnaire for a federal contract solicitation. The U.S. Department of Veterans Affairs is seeking a service-disabled veteran-owned small business to renovate clinical research laboratories at the Malcom Randall VA Medical Center in Gainesville, Florida. Interested contractors must submit past performance information demonstrating experience with similar design and construction projects by August 27, 2021 to the contracting officer. The questionnaire collects information about the contractor's performance on a previous project, including ratings in areas such as management, quality control, scheduling, and cooperation with government personnel. Reference contacts are asked to evaluate the contractor's performance and indicate if they would award another contract to the firm.
