1.0 INTRODUCTION

Terracon Consultants, Inc. (Terracon) conducted an asbestos survey of the sub-basement at the

Malcolm Randall Medical Center located in Gainesville, Florida.

The survey was conducted on August 12, 2020, by an AHERA accredited asbestos inspector in general accordance with our approved change order dated August 6, 2020. The inspection was limited to areas expected to be disturbed during forthcoming renovations of the Clinical

Laboratories on the floor above.

Terracon was accompanied during part of the survey by project engineers Michael Scot and Chris

Van Meter who are familiar with the areas likely to be disturbed. Pipes ducts and conduits in the designated area as indicated by Mr. Scott and Mr. Van Meter were surveyed and homogeneous areas of suspect asbestos-containing materials (ACM) were visually identified and documented.

Although reasonable effort was made to survey accessible suspect materials, additional suspect but un-sampled materials could be located in walls, in voids or in other concealed areas. Suspect

ACM samples were collected in general accordance with the sampling protocols outlined in United

States Environmental Protection Agency (USEPA) Asbestos Hazard Emergency Response Act

(AHERA) 40 CFR 763.86. Samples were delivered to an accredited laboratory for analysis by

Polarized Light Microscopy (PLM).

1.1 Reliance

This report is for the exclusive use of the client for the project being discussed. Reliance by any other party on this report is prohibited without written authorization of Terracon and the client.

Reliance on this report by the client and all authorized parties will be subject to the terms, conditions, and limitations stated in the proposal, this report, and the Agreement for Services

(AFS). The limitation of liability defined in the AFS is the aggregate limit of Terracon’s liability to the client.

2.0 BUILDING DESCRIPTION

The structure is a Veterans Administration Medical Center where renovations to the Clinical

Laboratories area are proposed. The sub-basement is the lowest level of the building and is below street level. Pipes, ducts, and conduits servicing the medical center are located in this area.

3.0 FIELD ACTIVITIES

The survey was conducted by Terracon AHERA accredited asbestos inspector Mr. Peter

Thompson. Copies of the laboratory accreditation, Mr. Thompson’s asbestos inspector certificate and other applicable licenses are attached as Appendix C. The survey was conducted in general accordance with the sample collection protocols established in USEPA AHERA 40 CFR 763.86.

A summary of survey activities is provided below.

Responsive ■ Resourceful ■ Reliable 2

3.1 Visual Assessment

Our survey activities began with visual observations of the sub-basement accompanied by two project engineers familiar with the project to identify homogeneous areas of suspect ACM. A homogeneous area consists of building materials that appear similar throughout in terms of color, texture, and date of application. Interior assessment was conducted throughout visually accessible areas of the structure. Building materials identified as glass, wood, metal or rubber were not considered suspect ACM. Concrete was not expected to be disturbed and was not sampled.

3.2 Physical Assessment

A physical assessment of each homogeneous area (HA) of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the USEPA as a material which can be crumbled, pulverized, or reduced to powder by hand pressure when dry.

Friability was assessed by physically touching suspect materials.

3.3 Sample Collection

Based on results of the visual observation, bulk samples of suspect ACM were collected in general accordance with USEPA AHERA sampling protocols. Samples of suspect materials were collected from selected locations in each homogeneous area within each facility. Bulk samples were collected using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker. The selection of sample locations and frequency of sampling were based on Terracon’s observations and the assumption that like materials in the same area are homogeneous in content.

3.4 Sample Analysis

Bulk asbestos samples were submitted under chain of custody to Moody Labs (Moody) of

Farmers Branch, Texas for analysis by polarized light microscopy with dispersion staining techniques (PLM) per USEPA methodology EPA 600/R-93/116. The percentage of asbestos, where applicable, was determined by microscopic visual estimation. Moody is accredited under the National Voluntary Laboratory Accreditation Program (NVLAP) Accreditation No. 102056-0.

The laboratory was instructed to analyze all samples.

4.0 REGULATORY OVERVIEW

The National Emissions Standard for Hazardous Air Pollutants (NESHAP) (40 CFR Part 61, Subpart M) regulates asbestos fiber emissions and asbestos waste disposal practices. The asbestos NESHAP regulation also requires the identification and classification of existing ACM according to friability prior to demolition or renovation activity. Friable ACM is a material containing more than 1% asbestos that, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure alone. Friable ACM is typically considered regulated asbestos containing material

(RACM).

The Florida Department of Environmental Protection (FDEP) incorporates the Asbestos NESHAP as reference in Rule 62-204.800 of the Florida Administrative Code. The owner or operator must

Responsive ■ Resourceful ■ Reliable 3 provide FDEP with written notification at least 10 working days prior to the commencement of asbestos abatement activities that will disturb asbestos-containing materials in total amounts greater than 160 square feet, 260 linear feet or 35 cubic feet.

The asbestos NESHAP regulation classifies ACM as either friable, Category I non-friable ACM or

Category II non-friable ACM. RACM includes all friable ACM, along with Category I and Category

II non-friable ACM that has become friable, will be, or has been, subjected to sanding, grinding, cutting or abrading, or ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder in the course of renovation or demolition activities. Category I non-friable ACMs are defined as asbestos-containing packings, gaskets, resilient floor coverings, resilient floor covering mastics and asphalt roofing products that contain more than 1% asbestos.

Category II non-friable ACM are all other non-friable materials other than Category I non-friable

ACM that contain more than 1% asbestos. Category II non-friable ACM generally includes but is not limited to cementitious material such as: cement pipes, cement siding, cement panels, glazing, mortar, and grouts.

The United States Occupational Safety and Health Administration (OSHA) asbestos standard for construction (29 CFR 1926.1101) regulates workplace exposure to asbestos. The OSHA standard requires that employee exposure to airborne asbestos must not exceed 0.1 fibers per cubic centimeter of air (0.1 f/cc) as an eight-hour time weighted average (TWA) and not exceed 1.0 fibers per cubic centimeter of air (1.0 f/cc) over a 30-minute time period known as an excursion limit (EL). The TWA and EL are known as OSHA’s asbestos permissible exposure limits (PELs).

The OSHA standard classifies construction and maintenance activities which could disturb materials which contain asbestos, and specifies work practices, training and other controls that employers must follow when engaging in each class of regulated work.

5.0 FINDINGS AND RECOMMENDATIONS

Twenty samples were obtained from eight identified homogeneous areas of suspect ACM associated with the work area.

Asbestos was identified in eight samples collected from four materials as part of this survey.

One of the identified materials was a mudded fitting in which black mastic was identified. After ruling out cross-contamination from the sampling activities it is possible that the mastic was either unintentionally mixed into the mud at the time of application or is contaminating the mud as falling debris from the mastic coating on the deck directly above the mud fittings. Consequently, mud fittings which may be disturbed in this specific area should be assumed to contain Category I non-friable asbestos material.

The black mastic on the underside of the deck, on paper wrapped pipe insulation, on insulated and uninsulated pipe fittings is a Category I non-friable material regulated asbestos containing material (RACM). Additionally, any other black mastic encountered in the work area on any surface should be assumed to be asbestos containing. These materials should be removed prior to disturbance by a State of Florida licensed asbestos abatement contractor in accordance with

Responsive ■ Resourceful ■ Reliable 4 a project design prepared by a licensed, accredited or certified Project Designer. Contractors who disturb these materials must be in compliance with OSHA regulations as contained in 29 CFR

1926.1101.

Materials not included in this survey may be revealed during the course of demolition activities.

These materials should be assumed to be asbestos-containing and should be sampled and submitted for laboratory analysis in order to determine their asbestos content before any further renovation or demolition activities that may disturb them.

6.0 LIMITATIONS/GENERAL COMMENTS

This asbestos survey was conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale. If other suspect materials or conditions are encountered, please contact

Terracon for proper evaluation and interpretations.

The results, findings, conclusions, and recommendations expressed in this report are based on conditions observed during our survey of the building. The information contained in this report is relevant to the date on which this survey was performed and should not be relied upon to represent conditions at a later date.

This report has been prepared on behalf of and exclusively for use by the client for specific application to their project as discussed. This report is not a bidding document. Contractors or consultants reviewing this report must draw their own conclusions regarding further investigation or remediation deemed necessary. Terracon does not warrant the work of regulatory agencies, laboratories or other third parties supplying information which may have been used in the preparation of this report. No warranty, express or implied is made.

RE: Report of Limited Asbestos and Paint Containing Lead Inspections, Polychlorinated

Biphenyls Inspection and Limited Sub-Basement Visual Observations

Malcolm Randall VA Hospital

Gainesville, Florida

Terracon Project No.: EQ197137

Dear Ms. Rigling:

The purpose of this report is to present the results of the Limited Asbestos and Paint Containing

Lead Inspections, Polychlorinated Biphenyls Inspection and Limited Visual Observations of a client-specified area of the sub-basement.

Terracon performed the pre-renovation survey on October 15, 2019, and October 16, 2019.

The limited surveys were performed based on information and drawings supplied by the client which determined the scope of work and the areas to be included in the inspections (designated areas).

Asbestos was detected in 30 of the 144 bulk samples obtained from the designated areas.

Lead was identified in one sample of paint collected from within the designated area as part of this survey.

Please see the attached report for details.

Terracon appreciates the opportunity to provide this service to RDC/John Poe Architects. If you have any questions regarding this report, please contact Mr. Peter Thompson at 904-900-6468.

LIMITED ASBESTOS AND PAINT CONTAINING LEAD INSPECTION,

POLYCHLORINATED BIPHENYLS INSPECTION AND LIMITED VISUAL

SUB-BASEMENT OBSERVATIONS

MALCOLM RANDALL VA HOSPITAL

GAINESVILLE, FLORIDA

November 22, 2019

Terracon Consultants, Inc. (Terracon) conducted a pre-renovation asbestos survey, a limited paint inspection for lead paint, polychlorinated biphenyls (PCB) inspection and limited visual observations of the sub-basement in the designated areas. Terracon performed the surveys on

October 15-16, 2019.

The limited asbestos survey, limited inspection of lead paint and limited PCB inspection was conducted by Mr. Ron Trapane and Mr. Peter Thompson, Asbestos Hazard Emergency

Response Act (AHERA)-accredited building inspectors. Copies of Mr. Trapane’s and Mr.

Thompson’s asbestos inspector training certificates are attached as Appendix D. The limited visual observations of the sub-basement were made by Mr. Peter Thompson.

The limited inspection of paint for lead was conducted by Mr. Peter Thompson. Mr. Thompson is an EPA-certified lead paint inspector.

The limited PCB inspection was conducted by Mr. Peter Thompson.

The limited surveys were performed based on information and drawings provided by the client.

The interior building components from the designated areas were surveyed and homogeneous areas of suspect asbestos-containing materials (ACM) were visually identified and documented.

Although reasonable effort was made to survey accessible suspect materials, additional suspect but un-sampled materials could be located in walls, in voids or in other concealed areas.

1.1 Project Objective

Terracon understands the purpose of these inspections was to provide information to the client regarding asbestos, lead, and PCB content in materials likely to be disturbed during renovations. EPA regulation 40 CFR 61, Subpart M, NESHAP, prohibits the release of asbestos fibers to the atmosphere during renovation or demolition activities. The asbestos NESHAP requires that potentially regulated asbestos-containing building materials be identified, classified and quantified prior to planned disturbances or demolition activities. Suspect ACM samples were collected in general accordance with the sampling protocols outlined in EPA AHERA regulation, 40 CFR 763.86. Samples were delivered to an accredited laboratory for analysis Polarized Light

Microscopy (PLM).

1.0 INTRODUCTION

Malcolm Randall VA Hospital ■ Gainesville, FL

Responsive ■ Resourceful ■ Reliable 2

The designated area was a portion of the existing basement. The level is below the first floor but opens to street level at the west side. Typical interior building components from the designated area consisted of drywall systems, concrete masonry unit (CMU) and mortar walls, various types of drop-in ceiling tiles and vinyl floor tiles (VFT), ceramic floor tiles and associated beddings, grouts, and thinset, cove bases (all with associated adhesives), fire stop putties, and caulks.

3.0 FIELD ACTIVITIES

The surveys were conducted on October 15-16, 2019. A summary of survey activities is provided below.

3.1 Asbestos Survey Activities

The surveys were conducted in general accordance with the sample collection protocols established in EPA regulation AHERA, 40 CFR 763.86. Our survey activities began with visual observation of the interior building components from the designated areas to identify apparent homogeneous areas of suspect ACM. A homogeneous area consists of building materials that appear similar throughout in terms of color, texture and date of application. The assessment was conducted throughout visually accessible areas of the designated work area. Building materials identified as glass, wood, metal or rubber are not considered suspect ACM.

Terracon lifted floor coverings in selected locations and did not observe additional floor coverings/layers except as noted in the report. However, as Terracon could not assess beneath all flooring areas, there may be isolated areas of additional suspect material present beneath floor coverings.

3.2 Physical Assessment

A physical assessment of each homogeneous area of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the EPA as a material which can be crumbled, pulverized or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect materials.

3.3 Sample Collection

Based on results of the visual observation, bulk samples of suspect ACM were collected in general accordance with AHERA sampling protocols. Random samples of suspect materials were collected in each homogeneous area. Samples were collected using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker.
