About this file

This is a Quality Assurance Surveillance Plan (QASP) for biohazard waste management services at the Whiteriver Service Unit (WRSU) in Whiteriver, Arizona. The QASP defines the Government's monitoring and evaluation approach to ensure contractor compliance with the Performance Work Statement, federal and Arizona regulations, and Indian Health Service policies.

The surveillance covers all biohazard waste management activities including collection, packaging, labeling, storage, transportation, treatment, and disposal across hospital and outpatient clinics, laboratory, pharmacy, dental, and radiology departments. Performance will be evaluated against OSHA 29 CFR 1910 (Bloodborne Pathogens), EPA/RCRA regulations, DOT 49 CFR Hazardous Materials Regulations, Arizona Department of Environmental Quality requirements, CDC infection control guidance, and IHS policies. The Contracting Officer's Representative (COR) conducts routine and incident-based surveillance coordinating with WRSU Infection Control, Safety, Environmental Services, and clinical departments. The surveillance approach emphasizes infection prevention, regulatory compliance, patient and staff safety, and environmental protection, with special attention to pickup reliability, storage conditions, and documentation accuracy due to WRSU's remote location. Key performance objectives include proper waste segregation (100% compliance, weekly visual inspections), timely waste pickup (no missed or delayed pickups, weekly schedule reviews), safe handling and transport (zero spills or violations, monthly observation), proper labeling and packaging (100% compliance, monthly random sampling), regulatory documentation (complete accurate manifests, monthly document review), and contractor staff training (current certifications, quarterly records review). Deficiencies are documented, contractors receive written notification and must submit Corrective Action Plans, and increased surveillance may be implemented. Performance data supports CPARS evaluations, award fee or incentive determinations, and contractual remedies for non-performance.
