KP Quality Assurance Surveillance Checklist
This checklist is to be completed by the Services Quality Assurance Representative (QA) no later than 1530 each day contract service is provided. All items are in accordance with the Performance Work Statement (PWS).
Kitchen Attendant Services: All kitchen personnel (KP), to include the supervisor, shall be assembled no later than 0700 in the front seating area of the dining facility, prepared to meet with Quality Assurance Representative (QA) to take attendance and receive safety briefing.

Contractor shall provide a full-time, primary and alternate onsite supervisor who is responsible for the performance of the work.
Designated contract supervisor shall be available during duty hours and should not be locked into a position as it restricts the ability to supervise, train and demonstrate required tasks. They will be available to maintain oversight, training and ensure full compliance with the requirements of the PWS. The supervisor may step in to correct shortfalls in the execution of the contract but may not function as a permanent individual assigned to a specific task. Supervisor should be available to discuss problem areas with QA representative or other designated personnel as assigned by the COR.
Contract supervisor shall utilize the contracted Quality Assurance Surveillance Plan that directly correlates to all areas within the PWS scope of work to meet the government’s satisfaction. The KP supervisor shall provide the completed QASP prior to 1530 as to give the government representative sufficient time to verify completion of all required tasks as outlined in the PWS.
The contractor shall provide all KP uniforms that are clean, neat and consist of long pants, short sleeve shirt, head cover/hairnet, and closed toe shoes with name tag. Contract manager and supervisor shall wear name tags that indicate job titles. *Personal clothing shall not be worn in addition to the required uniform worn during hours of operation.
All KP shall have photo I.D. and badge identifying name of contractor.
Contractor shall provide a current list of employed personnel. This list shall include completion/expiration dates of SERV SAFE training and medical certificates needed for Government inspection.
Contractor shall furnish a current employee roster/access list and submit to Security Forces within 7 days prior to scheduled work day.
All KP shall meet personal hygiene standards as listed in PWS.
All KP shall demonstrate sanitation practices to include changing gloves and washing hands when necessary.
KP shall distribute customer evaluation forms and assist in collecting forms while responding to customer questions/comments/concerns.
KP shall provide list of cleaning supplies/food service disposable items needed to be furnished for the following month; in addition to all cleaning supplies being adequately filled at all times (soaps, paper towels, toilet paper, eco-lab chemicals, paper plates, paper bowls, to-go silverware etc.)
KP shall set up tables and chairs in a formation as designated by either the DFAC Manager or COR. Once tables are arranged, condiment holders, napkin holders, and other applicable dispensers will be placed on the tables. If deemed necessary, tables covers will also be placed on tables prior to arranging the above listed holders and dispensers.
KP shall have all serving lines set up 15 minutes prior to scheduled serving time and shall not dissemble serving lines until 15 minutes after posted meal time (main line, short line, salad bar, beverage station, and ice cream) and in consultation with the COR.
All equipment and cleaning supplies will be furnished by the government. The contractor shall not bring in their own supplies. The contractor shall give a list to COR of any supplies/chemicals/consumables needed that they government does not have or needs to be ordered based on inventory and will be furnished by the government the following month.
KP shall serve and replenish all serving lines to ensure availability to patrons (main line, short line, salad bar, beverage station and ice cream).
KP shall set up Main line, Short line, and Salad bars, at least 15 minutes, but not more than 30 minutes before the start of the meal serving period. Arrange food on the lines as indicated on AF form 662 as stated in PWS.
KP shall provide sufficient serving utensils for all food serving lines.
KP shall provide sufficient serving personnel to ensure patrons will be served on main line at 5 patrons/minute and short line at 3 patrons/min.
KP shall clean up food or beverages spilled on floors, counters or elsewhere in the serving and dining areas within 4 minutes of the spill. If the spill presents a slipping hazard for government/contract personnel and patrons a wet floor sign will be immediately placed at the spill area.
KP shall clean/sanitize food service equipment/utensils to remove food particles, other soil, grease and cleaning or sanitizing residue at frequencies required by the PWS.
KP shall wash, rinse, sanitize and air dry tableware, trays, cookware and kitchen and serving line utensils. These items must be visibly clean from water spots, films and grease.
KP shall replenish tableware and dining table items to ensure availability to all patrons in accordance with PWS; to include the removal of items at the end of each RSD.
KP shall clean and sanitize dining room tables, chairs and benches to remove all foreign object, smudges, food particles and liquids. All dining room tables should be clean NLT 1400.
KP shall drain, disassemble, clean, de-lime and air dry all dishwashing equipment and remove foreign matter, lime deposits and smudges, in accordance with PWS. Remove curtains, spray arms, trays and other removable parts of clean and air dry.
KP shall wash, rinse and sanitize all food preparation equipment (work tables, mixers, peelers, sinks, tilt grill, stove burner grates, steamer and steam jacket kettle). If equipment was used, washed by KP and used again by a military member, that member will be responsible for cleaning. ***This excludes deep fat fryer***
KP shall clean ovens, ranges, and pass through food handling cabinets (hot & cold) daily to remove food particles and wash, rinse, and sanitize after each meal period.
KP shall clean serving line fixtures, windows, equipment, and all other equipment in the serving/dining area or which food is prepared, held, or dispended after meal.
KP shall clean coffee makers, milk dispenser, soda machine, ice cream machine and ice machine after each meal following manufacturer instructions; wash rinse, sanitize.
KP shall drain, clean interior, and remove rust and scale from hot food steam tables after each meal.
KP shall ensure kitchen and dining area floors have been vacuumed, swept and mopped with final hard service floor cleaning to be completed with the floor machine after every meal.
KP shall break down cardboard containers, and take to recycle bin; in addition to taking out trash and clean (wash, rinse, sanitize) soiled trash bins.
KP shall ensure all lavatories, locker rooms and vestibules in assigned food service facilities are cleaned and stocked with appropriate supplies (paper towel, toilet paper, hand soaps).
KP shall ensure areas around trash can storage and dumpsters are clean. KP shall sweep sidewalks and clean up around entrances and exits of dining facility after lunch as required. Cleaning includes removing trash, all paper and cigarette butts from area.
