Experience/Past Performance Questionnaire
Instructions: Offerors shall furnish the following information as it pertains to this request for proposal. Attach additional sheets as necessary.
EXPERIENCE WITH OHIO NON-NATIVE INVASIVE SPECIES:
Explain applicable experience (including personnel and resources used) performing identification of Ohio and/or regional NNIS as described in the Statement of Work. A majority of anticipated work in the IDIQ will target invasive trees (e.g. tree-of-heaven, princess tree, callery pear, Amur corktree) and woody invasive shrubs (e.g. autumn olive, privet, bush honeysuckle species, glossy buckthorn, winged burning bush, Japanese barberry). Additional treatments of invasive grasses (Chinese silvergrass, Phragmites, reed canarygrass, Japanese stiltgrass), invasive vines (kudzu, Oriental bittersweet, mile-a-minute, Japanese honeysuckle), and invasive sub-shrubs (e.g. multiflora rose, wineberry) may occur as needed.
Describe demonstrated proficiencies with identification of Ohio invasive plant species such as, but not necessarily limited to:
· Any applicable identification trainings attended
· Any applicable online or textbook identification resources used
· Any applicable in-house identification trainings conducted for employees
· Experience identifying Ohio NNIS plants during the spring/summer growing season
· Experience identifying Ohio NNIS plants in the fall
· Experience identifying Ohio NNIS plants during winter months
Overall experience and contractor capability are very important evaluation criteria for this contract. A contractor able to demonstrate years of satisfactory or above experience and the ability to capably perform a diverse range of scheduled line items will be given high consideration.
PAST PERFORMANCE ON SIMILAR PROJECTS:
Provide information on all projects completed within the last THREE (3) years that are similar in scope to the work described in the Statement of Work. The project can be federal, state or local government, as well as private contract. Specifically, provide the following:
· Name of the project;
· Whether your company was the general contractor or a subcontractor on the project;
· The total dollar amount of the project. If your company was a subcontractor on the project then provide your subcontracted dollar amount;
· Name of the customer/client, to include a point-of-contact and phone number,
· General overview of the project.
· Controlling similar invasive species, through use of physical, mechanical and/or herbicide treatments,
· Similar control techniques,
· Controlling invasive species in sensitive areas, to include: native vegetation; rare sensitive, or endangered species, as well as threatened species, and
· Equipment that was used on the project.
PAST PERFORMANCE NOTE: If an offeror has limited or no relevant project information, this should be addressed in the technical approach proposal. The Forest Service recognizes that an offeror may have minimal experience. In this instance, it is advised that the offeror address how they intend on insuring successful completion of the work, for example: subcontracting certain aspects of the project.
Past performance will be evaluated through reference checks provided in this section. Reference checks will be made by phone or email during the evaluation process. Reference checks will evaluate the contractors past performance with treatments similar to those in this contract.
