Past Performance Questionnaire

This questionnaire is designed to provide Defense Logistics Agency (DCSO-C3) with basic information regarding the Contractor’s past performance on previous contracts they have been awarded and the satisfaction with their performance.

Please send this Questionnaire to Contract Specialist(s) Christy Boyer at Christy.boyer@dla.mil.and Contracting Officer Alicia Barrowman at alicia.barrowman@dla.mil with the subject line SP8000-25-R-0022 NdFeB Magnet Block PPQ.
