About this file

This is a Past Performance Questionnaire template issued by the 628th Contracting Squadron at Joint Base Charleston, South Carolina for use in evaluating vendor performance on Aerospace Ground Equipment (AGE) Corrosion Control and Paint Services contracts. The questionnaire is designed to be completed by government agencies and customers who have previously contracted with prospective vendors to provide references on their performance capabilities for the ongoing AGE service requirement supporting the 437th Maintenance Group AGE Flight.

The questionnaire requests respondents to assess contractor performance across five key areas: ability to meet work requirements and quality standards, timely completion of orders within budget, responsiveness to quality issues and deficiencies, communication responsiveness, and overall performance rating. Evaluations are made using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) with requirements to provide detailed explanations for ratings other than Satisfactory, supported by documentation such as performance evaluations or discrepancy reports. The completed questionnaire must be returned no later than 4:00 PM EST on 15 April 2026 to Contract Specialists Morgan Hart (morgan.hart.1@us.af.mil; 843-963-8973) and Matthew Michel (matthew.michel@us.af.mil). Respondents provide information about the contracting activity, point of contact details, contractor information, contract numbers, contract type, performance period, annual and total contract values, and whether the contractor served as prime or subcontractor. The questionnaire also requests a determination of whether the respondent would award the contractor another contract and whether the contract experienced any terminations for default, convenience, or cause.
